Revolutions and evolutions in scientific endeavors have occurred throughout medical history, but the
greatest number and most notable in radiology have occurred in the past 30 years with the advent of new
electronics, computers, detectors, and contrast agents.
When crystal technology and radiographic tube design were primitive, there were theoretical questions
posed about the feasibility of real time or fluoroscopic CT. The conclusion in the early years was that it was
impossible to have sufficient photon production and unlikely that crystals could scintillate/reduce afterglow
enough to achieve such speed. And now the multidetector CT (MDCT) revolution
has begun! Current MDCT scanners have crystal detectors that are 99% efficient and have favorable
afterglow properties such that thousands of slices with superior spatial resolution can be achieved. Acquisition,
display, and processing of three-dimensional datasets are routine with separation of different vascular
phases during contrast administration. It is ironic that earlier technologies such as DSA were once
promoted as a replacement for CT, because the MDCT scanners can now totally replace catheter angiography
for evaluation of the vasculature for many parts of the body, including the lungs and heart. This is the
subject of this issue of the Radiologic Clinics of North America.
