MSCT coronary angiography effectively rules out CAD15 August 2007
MedWire News: Coronary artery disease (CAD) can be effectively ruled out in patients with suspected disease using multislice computed tomography (MSCT) coronary angiography, say French researchers.
This allows patients to be managed less invasively, by reducing the number of conventional coronary angiography procedures that are performed, they add.
Martine Gilard and colleagues from the Brest University Hospital carried out a prospective study to assess the reliability of MSCT coronary angiography to rule out CAD in 141 patients with suspected disease.
They initially enrolled 182 patients with a suspected CAD who were scheduled to undergo conventional coronary angiography but underwent MSCT coronary angiography instead.
Of these patients, 41 had inconclusive or abnormal MSCT coronary angiography results and so underwent secondary conventional angiography and were excluded from the study.
MSCT coronary angiography ruled out CAD in 141 patients, who were then followed-up for a mean period of 14.7 months.
During the follow-up period, none of the patients died, and 3.5% were referred for conventional angiography for recurrent or worsening chest pain. Only one patient (0.7%) experienced myocardial infarction after initial MSCT diagnosis.
The risks of subsequent death, new referral for conventional coronary angiography, or coronary events following MSCT coronary angiography diagnosis compare favourably with those for conventional coronary angiography, which were 0.4%, 4.3%, and 0.6%, respectively, the team notes.
Gilard and co-workers conclude in the Archives of Internal Medicine: "MSCT coronary angiography can safely be used alone to exclude a diagnosis of CAD in patients with suspected disease."
